$1,250 per table of 10, or $125 per person

* Includes two course sit down meal (main & dessert) and Beer & Wine beverage package.

Table lists & Dietary Requirements are due by 5.00pm Monday, 05 September 2022. Please note that entry to the venue may be refused if a table list is not provided. A link will be provided in the confirmation email once this form has been submitted.

The evening will include the HV Life Membership & Award of Merit inductions, together with presentations to players, umpires, coaches and administrators from the Victorian Hockey Community.

All Affiliates & Members of the Hockey Community are encouraged to attend.
